Key Responsibilities:

  • Program Development & Management 
    • Create and manage peer support group programs from inception to completion, including program structure, organization, execution, and coordination. Ensure all aspects are running smoothly and adapt as needed.
  • Group Facilitation
    • Lead peer support groups and workshops, both in-office and community-based, focusing on recovery and wellness.
  • Marketing & Outreach
    • Develop and execute strategies to promote peer support groups, including sourcing venues, creating marketing materials, and building community awareness.
  • Documentation & EHR Management
    • Maintain client documentation and update the EHR system within 24 hours of service delivery, ensuring compliance.
  • Record Keeping & Compliance
    • Ensure accurate record-keeping for all case management services, submit mileage reports on time, and maintain adherence to HIPAA standards and company policies.
  • Peer Support Services
    • Provide coaching, mentoring, and advocacy to individuals in recovery, assisting them in developing and achieving personal goals and navigating the behavioral health system.
  • Team Collaboration
    • Collaborate with a multidisciplinary team to develop individualized care plans and attend required supervision sessions.
  • Training & Development
    • Participate in ongoing training and professional development opportunities to enhance skills and knowledge.

 

Qualifications:

  • Education & Certification:
    • High school diploma or GED required; some college coursework in behavioral health is a plus.
    • Certification as a Peer Support Specialist from an AHCCCS-recognized program (required before hiring).
    • Valid CPR/First Aid certifications.
  • Licenses & Clearances:
    • Valid driver’s license, auto insurance, and a clean driving record.
    • Level One Fingerprint Clearance Card (required before hiring).
  • Health & Safety Requirements:

Position Overview:

Living and Balance Wellness Center, based in Phoenix, Arizona, is seeking a motivated and dedicated Therapist (or Therapist in Training) to join our team. This role is integral to providing therapeutic and counseling interventions to both youth and adults across varied settings: office, community, and telehealth. Ideal candidates will be in the process of accruing hours toward licensure and have practical experience in counseling, with a solid educational background in psychology or a related field.

Key Responsibilities:

Living & Balance Wellness Center is looking for a compassionate, diligent Group Facilitator to join our outpatient treatment facility. You will be working in conjunction with psychiatrists and other behavioral health professionals to provide direct care to assist with the treatment of clients who have various behavioral problems, such as substance abuse, post-traumatic stress disorders, mental instability, and physical and emotional abuse, under the care of a licensed administrator.

JOB SUMMARY:

  • Group facilitation (different group topics), psychoeducational learning, screenings, assessments, life skills, appropriate documentation daily, case management, crisis intervention, individual and/or group counseling services to clients daily, connecting with alternate resources, etc. Assists in maintaining a safe, orderly, and therapeutic environment. Provide and record patient medications, progress, and concerns, and assist in daily activities, whether therapeutic or recreational.

MINIMUM QUALIFICATIONS:

  • Minimum of a Master’s degree in behavioral health and one-year related experience in behavioral health; hospital, school, inpatient/outpatient facilities. *Candidates who have completed a doctorate degree, LAC, or LCSW with experience will be given preference. *
  • Experience in creating intake assessments, diagnoses, and treatment plans. (Required)

SUMMARY OF POSITION

The Behavioral Health Technician (Intern) is responsible for providing mental health assessment, treatment planning, counseling intervention services, and continually monitoring and documenting patient progress and outcomes. The Behavioral Health Technician (Intern) functions as a member of an interdisciplinary team, coordinating care and reporting patient progress and well-being to the clinical team and involved parties in accordance with the patient’s individualized treatment plan. As agreed upon, the Behavioral Health Technician (Intern) provides services to patients within the home, office, or community.

PRIMARY DUTIES

Under the supervision of an assigned site/clinical supervisor, interns will have opportunities to observe, learn and/or demonstrate various tasks related to counseling. These tasks include but certainly are not limited to

  • Empathetic communication with clients regarding addiction, mental illness, and recovery
  • Client admission and orientation process
  • Administration and interpretation of screening/assessment instruments
  • Development of treatment or service plans for individual, group, and/or family counseling sessions
  • Discharge planning
  • Referral/linkage to community resources, including social, legal, and educational providers
  • Interdisciplinary treatment team meetings
  • Clinical supervision sessions
  • Group facilitation includes life skills groups, psychosocial, psychoeducational, parenting, and domestic violence groups. Etc.

MINIMUM QUALIFICATIONS

  • Currently enrolled in an accredited Master’s degree in a license-eligible behavioral health field of Counseling.
  • Strong interest in working with individuals in recovery related to mental health and substance use needs.
  • Knowledge and experience in assessing and treating persons with mental illness and/or substance use disorders are preferred.

Living & Balance Wellness Center is looking for a compassionate, diligent Patient Navigator to join our outpatient treatment facility. The Navigator’s primary role is to enhance the likelihood of success, identify and reduce barriers for clients and connect them with needed services and resources.

JOB SUMMARY:

  • The Navigator will work with individuals who have been determined as in need of additional support and engagement and will be responsible for conducting outreach, engagement, case management, and ongoing wrap-around supportive services.  
  • The Navigator will also map out the most appropriate next steps for the individual who may need assistance accessing various services to achieve ongoing success. 
  • The Navigator is part of a multi-disciplinary team of internal and external stakeholders. 
  • The Navigator will document and track required patient interactions according to the Arizona Administrative Code (AAC), Arizona Department of Health Services/Department of Behavioral Health Services (ADHS/DBHS) Provider Manual, Commission on Accreditation of Rehabilitation Facilities (CARF), Community Bridges Policies and Procedures, and specific for the program.

Education, Experience, and Other Requirements:

  • A high school diploma or GED is required.  An associate’s Degree (or higher) in a field related to Behavioral Health is preferred but not required for this position.

  • 1-3 years of full-time Health Care related work experience where their prior experience would provide adequate exposure to both behavioral and medical crises is preferred but not required. 
  • 1-3 years of substance abuse/mental health recovery-oriented service is preferred but not required.
  • Or equivalent combination of education and experience.
  • Current AZ Driver’s License valid and in good standing.
  • Clear 39-month Motor Vehicle Record.
